REQUIRED FORMS FOR ALL SCHOLARSHIP RECIPIENTS

General Scholarship Financial and School Information Form

Please complete the front page of this form and forward it to the college or university you will be attending. This form provides us with the recipient’s information and where to send your scholarship payment. All payments are made directly to the school, in your name, before the start of the Fall semester. If your total scholarships exceed the cost of your tuition and fees, your scholarship(s) may be reduced to match your exact need. By signing the front page of the form, you are agreeing to these terms. The information on the back of the form is to be completed by the school. Depending on the policies of your school, the financial aid office will either complete and return the form to the Saginaw Community Foundation (SCF), or they will return it to you to send back to SCF. Please complete and return by July 15, 2024.

Note: If receiving multiple scholarship awards, complete an agreement form for each scholarship awarded. Instructions are available inside the form. Please complete and return the form(s) by July 15, 2024.
